1. Your Pastor's signature to approve your child attending camp.

2. The parent's signature in case of emergency

3. The student's signature, agreeing to the camp's rules, is located on the back of the form.

Not every question will be relevant to every child, so please mark N/A if it does not apply to your child. We want to support your child as best as possible during this week, and these questions will provide information on how to do so with your help and guidance.
